The Role

The role of an Aged Care Community Worker is multifaceted and essential in ensuring the well-being and empowerment of senior clients. This position is responsible for providing, one-on-one case management and community capacity building that supports seniors and carers from diverse Arabic-speaking countries to understand and access My Aged Care services.
This role is also responsible for fostering community connections and providing opportunities for shared learning and support.

 

About Arabic Welfare

Established in 1984, Arabic Welfare provides services to individuals and families from diverse Arabic speaking countries. We support our clients to realise their aspirations toward full participation in Australian life, and work closely with service providers to achieve this goal. Arabic Welfare staff are bilingual and bicultural, and our team includes qualified and experienced social workers, counsellors, youth and community development workers and Registered Migration Agents.

Responsibilities:

Case Management:

  • Perform an initial assessment of the client’s needs, concerns, and barriers to services.
  • Case management support through relevant approaches.
  • Maintain ongoing client support to help address identified issues and achievement of set goals.
  • Facilitate referrals and links to relevant services to support seniors and carers
  • Foster collaborative relationships with community service organisations, government, hospitals, specialist programs and other professionals and key stakeholders.
  • Participate in relevant meetings when required to support the needs of senior clients and carers.

Community Capacity Building:

  • Develop and evaluate initiatives tailored to target group needs.
  • Identify, support and proactively contribute to program developments, improvements and potential new initiatives.
  • Work collaboratively across AW teams to implement initiatives.
  • Build and strengthen relationships with community and faith leaders and stakeholders, which support the facilitation of potential partnerships and referrals.
  • Consult with diverse Arabic-speaking communities to understand local community issues and support the
    development of strategies to address them.
  • Plan and implement, educational group-based activities for the target group.
  • Identify emerging community needs to inform program delivery and development of potential
    service growth areas.
  • Support the development and implementation of capacity-building initiatives across AW
    service areas.

Project Implementation and Reporting:

  • Contribute to funding reports as required by Arabic Welfare.
  • Ensure the successful completion of work plan requirements and maintain accurate records of program delivery.
  • Maintain accurate and up-to-date client management and statistical records.
  • Participate in regular supervision and team meetings.
  • Proactively contribute to program developments and improvements.
  • Contribute positively to the organisation’s culture by developing and maintaining collaborative relationships with colleagues.

 

Knowledge, skills and experience

  • Experience in case management relevant to senior services to the newly arrived and well-established community.
  • In-depth knowledge of the welfare sector with a focus on My Aged Care and Senior Services will be highly regarded.
  • Experience in program development and implementation.
  • Ability to build strong relationships and engage with respected community leaders and groups.
  • Demonstrated ability to work in collaboration with other disciplines to support positive client outcomes.
  • Facilitation skills and experience in the delivery of information sessions and group work.
  • Well-developed communication skills, including written and verbal presentation skills.
  • Demonstrated ability to work as part of a team as well as independently.
  • Bilingual – Fluent in both Arabic and English.

Qualifications:

  • Relevant tertiary qualification in health or Community Services, such as Social Work, psychology, Community Development or related disciplines.
